Mechanism of action

What Teriparatid does in the body — step by step.

01

Teriparatide is the recombinant N-terminal fragment (1-34) of human parathyroid hormone (PTH).

02

It binds to the PTH-1 receptor on osteoblasts and osteocytes.

03

Unlike a persistently elevated PTH concentration (which promotes bone resorption), intermittent once-daily administration produces a predominantly anabolic effect on bone.

Research status

How far along is the evidence?

PRECLINICALEARLY CLINICAL STUDIESCLINICALLY ESTABLISHED
Teriparatide is a clinically well-studied, approved osteoporosis medicine.
Teriparatide is a clinically well-studied, approved osteoporosis medicine.
The pivotal trial (Neer et al., 2001) showed a significant reduction in vertebral and non-vertebral fractures versus placebo in postmenopausal women with osteoporosis.
Follow-up analyses documented a sustained reduction in fracture risk even after discontinuation.
Treatment duration is limited because of animal-model data (an osteosarcoma signal).
04

Known risks

Without human data, safety statements are limited. What is known:

01
Hypercalcaemia and hypercalciuria possible
02
Orthostatic hypotension, dizziness, especially at the start of treatment
03
Nausea, limb pain and headache described in studies
04
Osteosarcoma signal in long-term rat models — treatment duration limited
05
Contraindicated in bone malignancy, Paget's disease, pre-existing hypercalcaemia
