Mechanism of action

What Abaloparatid does in the body — step by step.

01

Abaloparatide is a synthetic analog of parathyroid hormone-related peptide (PTHrP 1-34).

02

It binds preferentially to the RG conformation of the PTH-1 receptor, which is thought to produce a more anabolically oriented, more transient signalling response than teriparatide.

03

Like teriparatide it is used intermittently (once daily) to favour bone formation over resorption.

Research status

How far along is the evidence?

PRECLINICALEARLY CLINICAL STUDIESCLINICALLY ESTABLISHED
Abaloparatide is approved as an osteoporosis medicine.
Abaloparatide is approved as an osteoporosis medicine.
The pivotal ACTIVE trial (Miller et al., 2016) showed a significant reduction in new vertebral and non-vertebral fractures versus placebo in postmenopausal women.
The ACTIVExtend follow-up investigated sequential alendronate after abaloparatide.
As with teriparatide, treatment duration is limited because of preclinical osteosarcoma signals.

Known risks

Without human data, safety statements are limited. What is known:

01
Hypercalcaemia and hypercalciuria possible
02
Orthostatic hypotension, dizziness, palpitations
03
Nausea and headache described in studies
04
Osteosarcoma signal in animal models — treatment duration limited
05
Contraindicated with increased osteosarcoma risk and bone malignancy
